Team Huddle: AI in Healthcare
A recent BMA survey found that over 40% of junior doctors are already using AI tools in some form — whether for study, clinical queries, or admin. The reality is most of us are using these tools daily but rarely talking about it openly. Starting this month, we're launching a new monthly AI in Healthcare session to change that. Led by myself and open to anyone interested, the aim is simple: introduce practical AI tools you can use on the ward, in clinic, and at your desk — and have an honest conversation about how they fit into modern practice. If you're curious, sceptical, or already a convert, there's a seat for you.
WEEK 1 • CTG
The Rising Baseline — More Than Meets the Eye

BACKGROUND A 28-year-old low-risk nullip presented twice to MAU with reduced fetal movements at 33+1 and 34 weeks. Antenatal CTGs met criteria both times but a requested ultrasound was never performed.
INTERPRETATION Intrapartum CTGs showed a progressively rising baseline with no decelerations — falsely reassuring. The pattern was driven by developing chorioamnionitis, worsened by multiple VEs and oxytocin augmentation.
OUTCOME Baby delivered at full dilatation but required 7–8 days on SCBU for significant neonatal infection.
💡 CLINICAL PEARL A rising baseline without decelerations should trigger consideration of infection, not just hypoxia. Oxytocin doubles HIE risk in the presence of chorioamnionitis — only start when the CTG is confidently normal. Limit unnecessary VEs and have a low threshold for septic workup.

WEEK 2 • GYNAE JC
Fezolinetant for Moderate-to-Severe Vasomotor Symptoms in Menopause (SKYLIGHT 1)

KEY FINDINGS Phase 3 RCT of fezolinetant, a neurokinin 3 receptor antagonist that blocks KNDy neuron overactivity in the hypothalamus to restore normal thermoregulation.
KEY RESULTS Fezolinetant significantly reduced hot flush frequency and severity versus placebo. Relevant given up to 80% of menopausal women experience vasomotor symptoms, with a third suffering over a decade. Offers a non-hormonal mechanism distinct from existing options like SSRIs/SNRIs.
TAKE HOME A promising non-hormonal alternative for women unsuitable for or declining HRT. Awaiting NICE adoption but already generating patient demand in menopause clinics.
WEEK 3 • GUIDELINE
NICE Guideline — Menopause: Identification and Management (NG23)
Presented by Niluka Kanaganayagam
Diagnosis: Clinical diagnosis in women over 45 — no blood tests needed. FSH only indicated under 45 (two readings, 4 weeks apart). Early menopause: onset 40–45.
Symptoms: Vasomotor (hot flushes, night sweats), genitourinary (dryness, dyspareunia, urinary discomfort), low libido, and subthreshold depressive symptoms.
HRT & POI: Trial of HRT is reasonable in symptomatic women over 40 even with regular periods. For suspected POI under 40, start standard HRT and refer — higher doses often needed. HRT preferred over COCP for cardiovascular and bone protection, but COCP acceptable if preferred.

Effect of HRT on Insulin Resistance in Postmenopausal Women

KEY FINDINGS Analysis from the WHI hormone trial examining the metabolic effects of HRT, specifically on insulin sensitivity via oestrogen receptor signalling in pancreas, liver, muscle, and adipose tissue.
KEY RESULTS HRT significantly reduced insulin resistance in postmenopausal women. Discussion emphasised that lifestyle intervention — exercise, diet, weight management — remains underutilised and arguably more sustainable, but is harder to implement and less commercially driven.
TAKE HOME HRT has metabolic benefits beyond symptom relief, but lifestyle modification must remain central. Medication can kick-start change but doesn't replace addressing root causes.
Historical Perspective
When Oxford Built a Computer to Read CTGs (1978)
1978
Long before ChatGPT or AI entered the mainstream lexicon, two Oxford professors were already asking the question: can a computer interpret a fetal heart rate better than the human eye? In 1978, Geoffrey Dawes and Christopher Redman began building an algorithm to do exactly that — objectively analyse the antenatal CTG. After more than a decade of development and thousands of traces, the System 8000 launched in 1991 as the first commercially available computerised CTG analysis tool. Today, refined using over 100,000 recordings, the Dawes-Redman criteria are woven into everyday UK practice through the NHS Saving Babies' Lives Care Bundle. As we launch our own AI in Healthcare sessions this month, it's worth remembering that AI in obstetrics isn't some distant future — it started down the road at the Nuffield, nearly half a century ago.
